The Community Collaboration Toolkit for Pennsylvania Providers is a Pennsylvania Perinatal Depression Project, an initiative of Pennsylvania Perinatal Partnership. It was funded by the Pennsylvania Department of Health and the U.S. Department of Health & Human Services, Health Resources & Services Administration.
